New School Sports Physical Requirements

The TTUSD and the NIAA have updated the requirements for children that will be participating in school sports. 

Starting grades 6th, 9th, and 11th: You will now be required to complete FORM B and FORM D.  This requires a physical exam by a medical provider.  (Of note, chiropractors are not able to complete these forms.)

Our physicians will be happy to complete this form during your child's Well Child Physical Exam.  Just call our office for an appointment.  In order for us to fill out the sports physical form during this visit, it is important that you bring the required forms and all immunization records to your child's appointment.  Be sure to complete your portion prior to your appointment!

Starting grades 7th, 8th, 10th and 12th:  In these intervening years, you'll need to fill out FORM E.  Any positive response to the questions on Form E will require a visit to a medical provider in order to be cleared to play.  If you have no positive responses, then no medical exam or physical is required to play a school sport.
